Waiver + Release

You must be 18 years or older to sign this form. 

• If you are under 18 years of age, a Parental/Legal Guardian must sign a Waiver and Release form only available at The Glute Garage studio that will be kept on file. 

• Those under 18 years of age will not be permitted to participate in a class without our waiver signed by the Parental/Legal Guardian on file. 

Individuals under 13 years old will not be able to participate in any The Glute Garage classes.

I understand that I, as the buyer, member, and or guest, I agree that if I engage in any physical exercise activity or use any equipment on the premises I do so at my own risk. I understand participation at this facility/property I assume all risk that might result including without limitations loss, injury, damage or theft of any personal property, with or without use of a personal trainer, I agree on behalf of myself, my personal representatives, my heirs, my executor, my administrators, my agents and assigns to release and discharge us and our affiliates, employees, agents, representative, successors and assigns from mly and all claims or causes of action known and unknown arising out of negligence.

 

This is a waiver and release of liability and includes, without limitations, injuries which occur as a result of:

 

1. My use of any equipment or facilities which may malfunction.

2. Om maintenance of any equipment or facilities.

3. Our instruction or supervision.

4. My slipping or falling while in the health club or anywhere on the premises.

 

I have acknowledged I have read this waiver and release and fully understand that it is a release of liability, I am waiving any right that I may have to bring any legal action or claim against you of your facility for any reason.

 

Medical Release

I have enrolled in a program of progressive physical exercise, which may include but not be limited to aerobics and or weight training. In consideration of my participation in  THE GLUTE GARAGE LLC program, myself, my heirs and assigns, hereby release THE GLUTE GARAGE LLC, Maria Londono, her heirs, associates and employees from any actions arising from participation in these programs. I hereby affirm that I am in good physical condition. I do not suffer from any disability that would limit and preclude any exercise program. The possibility of certain unusual changes during exercise does exist. All members are strongly urged and recommended to have a complete physical examination by a medical doctor prior to beginning any program of strenuous, new activity. I have been informed of the need for a physician's approval for participation in a program of progressive exercise. I fully understand the strenuous nature of the program.

 

I accept the complete responsibility for my health and well-being in this voluntary exercise program and hereby release THE GLUTE GARAGE LLC, Maria Londono, their heirs, associates and employees from any liability in the event of injury, however caused, occurring during or after my participation in this program. 

 

I give THE GLUTE GARAGE LLC permission to use any images and video taken in various publications and promotional materials.
